PRE-KINDERGARTEN ENROLLMENT INFORMATION

The Carrollton-Farmers Branch ISD offers a pre-kindergarten program based on state guidelines for eligible four-year-olds.

To be eligible for the 2009 - 10 school year:

A child must be four (4) years of age on or before September 1, 2009

Meet one of the following criteria:

• Be eligible by income
• Demonstrate limited English proficiency (verified by district testing).

To register parents need to bring:

• An original birth certificate (no copies please)
• Original Social Security card (no copies please)
• Current immunization record (must have 4 year old shots)
• Proof of residency-
  - Lease agreement or mortgage coupon
  - A current utility bill with name and address (gas, water, electric)
  - Driver’s license or identification of parent or legal guardian
• Emergency phone numbers for home, family physician, dentist and three personal emergency contacts.

**If you reside in a home of another person, before you can register, you will need to:

1. Obtain a notarized letter from that person stating that you and your child live with them.
2. Please take the notarized letter along with the current utility bill to Student Services at 1820 Pearl Street Bldg B. (972) 968-6500
