New Student Registration

Thursday, August 14, 2008 --> 8:30 - 11:30 and 1:00 - 3:00

Please provide the following information for registration for the school year:

1. Birth Certificate

2. Up-to-date immunization records

3. Social Security card

4. Photo ID (driver's license)

5. Lease or contract (mortgage) of house/ apartment

**If you are living with someone, you must go to the Office of Student Services, and they will provide you with a necessary form prior to coming to our school. (see below)

6. Two (2) proofs of residency with the same address as lease or contract (mortgage):

    Ex: Utility bills (electricity, water, phone)

7. If your child attended school last year (2007-2008), please provide school information -- we will send for his/her records.
