TEA FORM	TEA forms issuded druing the school year expire 30 days from issuance. TEA forms issued the last 5 days of the school year expire the first day of the following school year. TEA Forms will NOT be available June 24 to July 15.

SENIORS	Final transcripts are NOT automatically mailed to colleges. Students are responsible for requesting a final transcript be sent to their college. You may order a final transcript online at www.Docufide.com or follow the link of the Creekview website to the Docufide website. The transcript fee is $5.00 per transcript. The transcript from Docufide does not include SAT or ACT score. If you did not have your scores sent to the college at the time the test was taken, you may order SAT test scores from www.collegeboard.com and ACT test scores from www.actstudent.org Transcripts will NOT be available from the records office June 24 to July 15.

MOVING	Students who are planning to move over the summer should contact the Records Office at 972-968-4814. Final report cards and registration packets for the 2008/09 school year will be maied to the address we have on file.

NON-RETURNING STUDENT	If you are not returning to Creekview for the 2008/2009 school year, please stop by the Records Office during the summer office hours and complete a "Non-Returning Notification Form". This is not necessary for graduates.
