This is the De Witt Perry School Nurse website. My name is Amber Cichocki, RN, CPN (Registered Nurse, Certified Pediatric Nurse). It is my 5th year as a full-time nurse for the Carrollton Farmers Branch, Independent School District and my 5th year at DeWitt Perry Middle School. My first nurse training included working with adult cardiac and renal patients in the hospital setting. I then switched my focus to pediatrics (children). I have worked in the Neo-Natal Intensive Care Unit, General Pediatric surgery unit, a pediatric Neurology clinic and as a Pediatric Orthopedic Admissions nurse. This webpage is to assist you. Immunizations must be kept current to remain enrolled in school. Please make a mental note that state law has changed this year. All 6th graders before entering the 7th grade will now need a Tdap, chickenpox and a Meningitis vaccination before they will be allowed in school. Please remember to send a copy of the immunization with your student to the nurses office whenever you have updated your immunizations. Medication

Click here for District Policy Click here for District Medication form ALL medications are to be turned into the nurses office. There is paperwork that is needed to be completed by the guardian or parent before medication can be given. All medications are to be in their original container and properly labeled. The pharmacy label must state the student's name, medication, dosage, doctor's name and date prescription was filled. The prescription is to be current within the last 12 calendar months. Any changes in prescription medications require either a new prescription labeled bottle or written physician request for dosage change. Non-prescription medicines should have the student's name affixed to the original bottle. A new parental permission request is to accompany any change in medication. Cough drops are considered as medicine.
For all students who are new to the CFB-ISD district, vision, hearing, screenings are completed. All 7th graders have these screenings: hearing, vision, and acanthosis nigricans (a skin condition that can help identify persons who run the risk of developing diabetes in the future). All 8th graders have these screenings: scoliosis. If you have questions or feel that your student may need to be screened on any or all of these health issues, please contact the nurse's office. Also beginning this year, all students will have a current BMI (body mass index) calculated. A parent may request to receive the results.
Your student should stay home when they have a fever of 100.4 F or higher. Or if they experience diarrhea (bathroom more than two times), vomiting, red draining eyes, draining ear or undiagnosed skin rash or skin condition (draining sores, bumps, constant itching etc.) they should stay at home. If they have been diagnosed by a doctor with having chicken pox, measles, mumps, infectious hepatitis, impetigo, herpes, scabies, lice, MRSA, strep infection, influenza or any other communicable disease, please keep them home until released by the doctor. Please notify the school by phone of their absence and the reason why they are absent. For example: "Johnny is not in school today because the doctor said he has strep throat or Johnny is as home today because he has a fever." By keeping your student at home when they are ill you help control and prevent the spread of communicable diseases to others. After your child has recovered from an illness, they should be free of symptoms and the fever should normal for 24 hours without the use of medicine before returning to school. If a student becomes ill at school, or has other medical related issues, we will make every possible effort to reach you. It is important for you to supply us with accurate phone numbers of your home, mobile, and work. It is also important to have people on our files whom we may contact in case you can not be reached. This is for the protection and safety of the student.
