Supplemental Life
(For New Employee Enrollment Only)
Enrollment in the
supplemental plans must be completed by the 30th day
following your date of hire. This booklet contains
information on each plan.
Applications for each plan are included with the
specific plan description and must be completed in
ink.
Guaranteed coverage amounts will be effective on the
eligibility date for your benefits. For coverage
subject to underwriting guidelines, premiums will
be deducted when the district is notified of the
application approval.

Voluntary
Term Life Insurance
- Supplemental life
insurance is underwritten through MetLife.
- Coverage is
available for the employee, spouse and dependent
children.
- Employees must apply
for a minimum of $20,000, with a maximum amount
(Including all group life insurance)
of seven (7) times your annual salary not to exceed
$500,000.
- Spousal coverage must
be for a minimum of $5,000 or a maximum of 50% of
the employee benefit not to exceed $100,000.
- Coverage for dependent
children can be chosen in amounts of $1,000 units
from $2,000 to $10.000.
All children in a family must be covered for the
same amount.
- The coverage is
portable, which allows you to continue the coverage
if your employment status changes with the district.

- For new hires, thee is
a guaranteed issue of 7 times your annual salary up
to $150,000 for employees. 50% of employee
benefit
up to $50,000 for spousal coverage and maximum of
$10,000 for dependent children
Coverage amounts above those limits for employees
and spouses re subject to underwriting
and require completion of a different application.
