Enrollment Instructions
 

Enrollment in the supplemental plans can be made during the benefits enrollment period of October 1 – 31.  Procedures for the cancer plan are:

·         Application for the cancer/specified disease plan is in the handout and must be completed and returned to the benefits office by October 31for anyone wishing to add new coverage or increase or decrease current coverage.  Current coverage will continue unless a change is made. 

·         For those employees covered by Hartford, there is no change in benefits or rates for 2008.  Hartford is not open for new enrollment.

·         Benefit representatives will be available the week of October 27 7:30-4:30 at the Benefits Office at the Administration Building annex to assist with enrollment and answer questions.  Representatives from the cancer plan will also be available at the Benefits Office from 1:30 – 4:30 the week of October 27.

 

Cancer/Specified Disease/ICU Coverage

·         The coverage is underwritten by National Union Fire Insurance Company of Pittsburgh, through AIG Companies and includes coverage for cancer and specified diseases, with an optional rider available for intensive care.

·         There are no changes in benefits for 2009.  Current coverage will continue unless a change is made during the enrollment period.

·         There will be no rate change for 2009.  Rates are available on application in the cancer packet.

·         Policy pays directly to you regardless of other coverage and benefits received are non-taxable.

·         The coverage is portable if your employment status with the district changes.

·         Coverage is available for the employee and dependents and subject to health questions and underwriting approval.
