Unum Disability Income Protection
(For New Employee Enrollment Only)

Enrollment in the supplemental plans must be completed by the 30th day following your date of hire.  This booklet contains information on each plan. 
Applications for each plan are included with the specific plan description and must be completed in ink.
Guaranteed coverage amounts will be effective on the eligibility date for your benefits.  For coverage subject to underwriting guidelines, premiums will
be deducted when the district is notified of the application approval.

Salary Protection Plan

• The income protection plan, underwritten by UNUM Life Insurance Company of America, provides a monthly benefit
  should you be unable to work during a period of certified total disability due to a personal illness or accident.
