Procedures for Student Enrollment

Basic requirements for student enrollment include:

• Copy of birth certificate

• 2 Proofs of residency (Lease agreements, phone, electric or gas bill)

• Original health card

• Copy of Social Security card

Parents or guardians will complete a registration packet including the following:

• AGR card

• Emergency Information Card

• Home Language Survey

It is most important that current daytime phone numbers are on file at all times. Please provide the office with this information whenever a change occurs.
