New-Student Enrollment Information |

Enrollment of new students is by appointment only.
Parents can pick up an
enrollment packet from the school receptionist, who will schedule an
appointment for registration; or, parents can call 972-968-3700 to make
an appointment. Please arrive 30 minutes before the appointment to
fill out necessary paperwork.

These documents must be originals.
We cannot accept facsimilies (FAX) of these documents from other schools. |
* Proof of Residency
-- Current Apartment lease showing child is an occupant
or a Mortgage statement
-- 2 current utility bills with same address as lease
or mortgage
Electric, gas, or water bills
only--we cannot accept cable or phone bills |
| * Withdrawal paperwork from
last school student attended including last Report Card |
| * Student Immunization Records |
* Student Birth Certificate
* Student Social Security Card (or Passport if new to USA) |
Complete Enrollment
Packet includes the Following: |
- 2 sets of Registration Forms signed by Parent (both forms
must be filled out)

- Home Language Survey signed by Parent (2 pages)
|
- Previous School Information Survey
|
- Survey that will determine Placement for Student
|
- Permission to Administer Medication
|
